Fraser is looking for Case Managers to work with individuals on the CADI & BI waivers in Hennepin, Ramsey, and Dakota counties. Case Managers serve as an advocate to Fraser clients and connects clients to resources and services within the community. The ideal candidate will demonstrate compassion for clients, commitment to success and value relationships. We seek an individual who is very well organized, has experience with individuals with developmental or intellectual disabilities and/or mental health diagnoses and experience documenting and case noting.
Bilingual candidates encouraged to apply! Responsibilities: The Case Manager provides client-centered case management services to individuals receiving CADI/BI waiver services. Case Managers typically have about 40-45 clients on their caseloads and meet with clients three times per year or once every four months. The Case Manager is responsible for care coordination, monitoring health and safety needs of clients, monitor services to ensure they are effectively meeting the client’s needs and intervening as necessary. Caseloads are typically a majority of adults, with a small mix of children as well. Fraser offers:
